This policy explains what information this website collects, why, and what we do with it. It covers metamorphosismedspa.com only.
The short version
- We do not use advertising trackers, analytics pixels, or third-party advertising cookies on this site.
- Messages sent through our contact form are emailed to the practice and are not stored in this website's database.
- If you join our newsletter, we store your email address and, if you give one, your first name — nothing else.
- Please do not send medical information through this website.
Information you give us
Contact form
Our contact form asks for your name, email address, an optional phone number, and your message. When you submit it, that information is sent to the practice by email. It is not written to this website's database.
Our systems keep a short technical log recording that a message was submitted, when, and whether it sent successfully. That log contains a one-way scrambled value derived from your network address, used only to detect abuse of the form. It does not contain your name, email address, phone number, or the content of your message.
Please do not include medical details. The form asks you not to send symptoms, diagnoses, or health history. Email is not a secure channel. Anything clinical is collected securely at your consultation instead.
Newsletter
If you sign up for our newsletter we store your email address, your first name if you provide one, whether you signed up from the footer or the signup page, and the date. We do not record which treatments you viewed or link your subscription to any patient record. You can unsubscribe at any time using the link in any email we send, or by contacting us.
Information collected automatically
Like most websites, our server records standard technical information when a page is requested, such as the address of the page, the time, your browser type, and your network address. This is used to operate and secure the site.
We do not run Google Analytics or any comparable analytics product on this site, and we do not host advertising or social media tracking pixels.
Third parties
A small number of outside services are involved in running this site. When your browser loads content from them, they necessarily receive your network address.
- Google Fonts — supplies the typefaces used across the site.
- Google reCAPTCHA — protects the contact form from automated abuse. It is present only on the contact page.
- Cherry — provides the payment plan tool. It appears only on our Payment Plans page. If you apply for financing, you deal with Cherry directly and their own privacy policy governs that application. We do not receive or store your financial details.
- Amazon Web Services — hosts the images on this site.
Links to Facebook and Instagram are ordinary links. Nothing is embedded from those services, and they are not notified when you visit our pages.
How long we keep things
Contact form emails are retained in the practice's mailbox under our normal correspondence practices. Newsletter subscriptions are kept until you unsubscribe. Technical logs are short-lived and kept only for security and troubleshooting.
Your choices
You can unsubscribe from our newsletter at any time. You can ask us what information we hold about you, ask us to correct it, or ask us to delete it. To make a request, contact us using the details below. Depending on where you live, you may have additional rights under your state's privacy law.
Children
This website is intended for adults. We do not knowingly collect information from children under 13 through this site.
Security
This site is served over an encrypted connection. No method of transmission over the internet is completely secure, which is why we ask you not to send health information through the contact form.
A note about your medical records
This policy covers the website. It is not our Notice of Privacy Practices, which is the separate document describing how the practice handles protected health information under HIPAA. Ask us at the front desk for a copy.
Changes
If we change this policy we will update the date shown at the top of this page.
