Lip Filler: What to Expect Before, During and After Treatment

Lip filler is one of the most requested treatments at any med spa, and one of the most misunderstood. Done well, it can restore volume lost with age, balance an uneven lip or add subtle definition without looking "done." Knowing what to expect before, during and after treatment makes the experience easier and helps you get the best result. Here is a step-by-step guide from our Downers Grove med spa.

What is lip filler?

Most lip fillers are made from hyaluronic acid (HA), a sugar molecule that occurs naturally in the skin and holds onto water. HA fillers such as Restylane and Juvéderm come in different formulations. Softer, more flexible gels are used for subtle hydration and fine lines, and firmer gels are used for structure and definition. Most also contain lidocaine, a local anesthetic, for comfort.

Lip filler can be used to:

• Add volume to thin lips or lips that have lost fullness with age
• Define the border of the lip (the vermilion border) and the Cupid's bow
• Correct asymmetry between the upper and lower lip or between sides
• Soften fine vertical lines around the mouth ("lipstick lines")
• Hydrate lips for a smoother, healthier look

Before your lip filler treatment

The consultation

A good result starts with a conversation. Your injector will look at your lip shape, facial proportions and the balance between your upper and lower lip, and talk through what you want. Bringing photos of lips you like is helpful, but the aim is a shape that suits your face. If this is your first time, starting conservatively is usually the best approach, as you can always add more later.

Preparing for your appointment

• Reduce bruising risk. If your doctor agrees, avoid aspirin, ibuprofen, fish oil, vitamin E and other blood-thinning supplements for about a week beforehand. Never stop prescribed medication without talking to your doctor.
• Skip alcohol for 24 hours before treatment.
• Tell us about cold sores. Injections can trigger an outbreak, so we may recommend an antiviral beforehand.
• Plan around events. Book at least 2 weeks before a wedding, photos or vacation to allow swelling to settle.
• Avoid dental work for about 2 weeks before and after treatment.

Lip filler is not recommended during pregnancy or breastfeeding, or if you have an active infection or cold sore on or near the lips.

During treatment

After cleansing, a topical numbing cream is applied for about 15 to 20 minutes. Your injector then places the filler in small amounts using a fine needle or cannula, shaping as they go and checking symmetry from several angles. With numbing and the lidocaine in the filler, most people describe the sensation as pressure with brief pinches rather than pain.

The injection itself usually takes 15 to 20 minutes. Afterward, you will see your new shape straight away, although it will look fuller than the final result because of swelling.

After lip filler: the recovery timeline

The first 48 hours

Swelling is normal and usually peaks on the day after treatment. Lips can look larger or slightly uneven, and they may feel firm or lumpy. This is swelling, not your final result. Bruising at the injection points is also common.

Days 3 to 7

Most of the swelling settles and your lips soften. Any bruising fades. Mornings may still show a little more puffiness.

Week 2

The filler has integrated with your tissue, and this is your final result. We recommend a follow-up at around 2 weeks to review the result and make any small refinements.

Aftercare tips

• Apply a cold compress, wrapped in a cloth, for short periods on the first day.
• Avoid strenuous exercise, saunas, hot yoga and alcohol for 24 to 48 hours.
• Do not press, massage or pout at the area unless your injector tells you to.
• Sleep with your head slightly elevated for the first night.
• Skip lipstick for the first day, and avoid kissing and drinking through a straw on the day of treatment.

When to call us

Serious complications are rare, but it is important to know the warning signs. Contact the practice straight away if you notice severe or increasing pain, white or blotchy skin, dusky discoloration, or any changes in your vision. These need prompt assessment.

How long does lip filler last?

Most lip fillers last 6 to 12 months. Lips move constantly, so filler breaks down faster there than in areas such as the cheeks. The exact duration depends on the product, how much is used and your metabolism. Many people find results last longer with repeat treatments.

A major advantage of hyaluronic acid filler is that it is reversible. If needed, an enzyme called hyaluronidase can dissolve it.

Timing lip filler around events and the weather

Allow at least two weeks between treatment and any big occasion, so swelling has settled and there is time for a follow-up tweak. For wedding season in the spring and summer, that means booking early.

In winter, cold wind can leave lips chapped. Come to your appointment with lips well moisturized, and let us know about any history of cold sores, which cold, sun and stress can all trigger.
