Why faces lose volume
From our mid-twenties, the body makes around 1% less collagen each year. At the same time, fat pads in the face shrink and shift, and the bones of the face slowly change shape. The result is hollowing at the temples and cheeks, deeper folds around the nose and mouth, a less defined jawline and thinner, crepey skin.
How does Sculptra work?
Sculptra's active ingredient is poly-L-lactic acid, a biocompatible material that has been used in medicine for decades, including in dissolvable stitches. When it is injected into the deeper layers of the skin, the PLLA microparticles act as a scaffold and trigger a natural response: your body lays down new collagen around them. Over the following months the PLLA itself is gradually broken down and absorbed, leaving your own collagen behind.
This is what is meant by collagen stimulation. Because the new support is your own tissue, results build slowly and look like a well-rested, healthier version of you rather than an obvious "done" look.

The two are not competitors. Many patients use Sculptra for overall structure and a hyaluronic acid filler for precise detail work, such as lips or under-eye hollows.
The Sculptra treatment timeline
Your first session
After a consultation and facial assessment, the skin is cleansed and numbed. Sculptra is placed with a fine needle or cannula in the areas that need support, often the temples, cheeks, jawline and around the mouth. The appointment takes around 30 to 45 minutes.
Immediately after
You will look fuller straight away, but this is mostly the water used to mix the product. It is absorbed within a few days, and your face returns to roughly how it looked before. This is expected, and it is not the result.
The 5-5-5 rule
To help the product spread evenly, you will be asked to massage the treated areas for 5 minutes, 5 times a day, for 5 days. It is simple but important.
Weeks 4 to 12
Collagen production ramps up and you start to see gradual improvement. Your second (and, if needed, third) session is scheduled around 4 to 6 weeks after the first, so each one builds on the last.
Month 3 onward
Most people see their full result about 2 to 3 months after the final session. Friends tend to say you look well rested rather than asking what you have had done.
How long do Sculptra results last?
Sculptra is one of the longest-lasting non-surgical treatments available. In clinical studies, results lasted up to 2 years, and many patients maintain their results for longer with a single top-up session every 18 to 24 months. Because you are rebuilding collagen rather than temporarily adding volume, the improvement fades slowly rather than disappearing.
The aging process continues, of course. Sun protection, not smoking and a good skincare routine all help protect the collagen you have built.
Who is a good candidate for Sculptra?
Sculptra suits adults who:
- Have mild to moderate volume loss or hollowing
- Want a gradual, natural-looking change rather than an instant one
- Prefer fewer treatments with longer-lasting results
- Are interested in improving skin quality and firmness, not just filling lines (see also skin tightening)
It is not recommended during pregnancy or breastfeeding, for people with active skin infections in the treatment area, or for anyone prone to keloid scarring. Your physician will review your medical history first.
Side effects
Swelling, tenderness and bruising at the injection sites are common and usually settle within a few days. Small bumps under the skin (nodules) are the main known risk. Careful technique, correct dilution and the 5-5-5 massage all help to reduce it.
Planning Sculptra ahead of an event
Sculptra is the opposite of a quick fix: with two or three sessions spaced weeks apart and results that build over the following months, plan to start four to six months before a date that matters. A series begun over the winter typically reaches its full effect in time for spring and summer weddings and reunions.
